Mayerson JCC Membership: Cancellations must be submitted through this form no later than the 20th of the month prior to your intended cancellation date (for example, cancellation requests received on or before July 20th will result in cancellation as of the end of business on July 31st). Requests received after the 20th will be processed for the end of business on the last day of the following month, and applicable dues will remain in effect through that period.

Group Swim Lessons, Small Group Training, and Pilates Group Training: Cancellation requests must be received no later than the end of business on the first day of the last month you wish to participate (for example: cancellation requests received on or before July 1st will result in cancellation as of the end of business on July 31). I would still be responsible for my final month(s) payment(s) and may participate in the class for the period which I have paid. Cancellation requests received on or after July 2 would result in cancellation effective at the end of business on August 31.
